Reporting to the Director, High Blood Pressure Strategy and Professional Education and working closely with the Senior Specialist, Outreach and Practice Support and the Manager, Smoking Cessation in Primary Care, the Outreach and Practice Support Specialist is responsible for the clinical coaching, integration of Quality Improvement (QI) and change management principles and the coordination of activities associated with the Hypertension Management Program (HMP) and the provision of back up support for the Ottawa Model For Smoking Cessation in Primary Care Program in primary care sites, to ensure the effective implementation of  the tools and interventions associated with the programs. The HMP includes both the main Hypertension Management Program and the Aboriginal Hypertension Management Program. A major component of this work will involve the building and management of both internal and external relationships with the multidisciplinary team of healthcare providers at involved sites.  

The development and application of operational skill sets in areas such as knowledge exchange and adult behaviour change principles will be instrumental in identifying/reporting any barriers and enablers to achieving overall project outcomes.

Key Accountabilities:

Relationship Development and Program Management:

  • Develop and manage healthcare provider relationships at HMP sites as appropriate, including assisting in the adoption and use of tools and interventions developed as part of the HMP, in collaboration with the  Senior Specialist, Outreach and Practice Support
  • Facilitate clinical coaching, quality improvement and change management at all sites, including the uptake of tools and interventions via site visits and academic detailing.  Also gather feedback from all participants at all assigned sites (including administrative staff) and provide them with support
  • Work with the Manager, Smoking Cessation in Primary Care and provide backup support for the Ottawa Model For Smoking Cessation (OMSC) in Primary Care Program

Inter-professional communications:

  • Facilitate communication among all involved healthcare professionals
  • Assist in design and implementation of inter-professional communication tools and interventions

Additional HMP Responsibilities:

  • Assist in the team’s development, organization, implementation, and evaluation of HMP tools, interventions, and programming
  • Collect and analyze information from all sites to identify and drive quality improvement in implementation

Additional OMSC in Primary Care Program Responsibilities:

  • Assist in the organization and facilitation/practice support of OMSC in Primary Care Program interventions and programming


  • Communicate (verbal and written) and collaborate effectively with internal/external team members and stakeholders
  • As with Relationship Management above, develop and maintain strong working relationships with healthcare professionals and team members

  • Potential resistance to uptake of tools and interventions (e.g. available time issues)
  • Project is dynamic with potential for frequent changes

Other Requirements:

  • Has strong clinical background in a related discipline (such as nursing or allied health)
  • Strong knowledge and experience of adult learning principles, quality improvement and primary healthcare issues
  • Knowledge of hypertension (and tobacco cessation) would be assets
  • Knowledge or experience working in multi level disease organization would be beneficial
  • Proficient in Word, Excel and PowerPoint
  • Driver’s license required
  • Significant provincial travel required
