Health Promoter - Diabetes Prevention Project

The Heart Connection, in collaboration with the Haliburton, Kawartha, Pine Ridge District Health Unit, requires a Health Promoter to co-ordinate the Workplace Wellness Program of the Diabetes Prevention Project in Northumberland County and the City of Kawartha Lakes. The Heart Connection involves community partners working together to help people become healthier by encouraging them to eat well, be active and stay smoke-free.

The Health Promoter is responsible for planning, implementing and evaluating the Diabetes Prevention Project. The position involves collaboration with workplaces to facilitate health promotion and the development of healthy workplace policy.

This position is funded by a financial contribution from the Canadian Diabetes Strategy, Population and Public Health Branch, Ontario Region, Health Canada.

The successful candidate will have:

a baccalaureate degree in health promotion/education or equivalent

demonstrated skills in group facilitation, community mobilization, social marketing, healthy public policy development, and behaviour change education

excellent organizational and analytical skills

skills in conducting needs assessment/research, program planning and evaluation

excellent interpersonal, oral and written communication skills

a valid Ontario Driver's licence and access to a vehicle

experience in basic computer applications in a Windows-based environment (WordPerfect as a minimum)

experience in public/community health, employee health, or health promotion is desirable

Location: Port Hope, with travel throughout Northumberland County and the City of Kawartha Lakes

Salary Range: ONA $44,843 - 51,085 (2001) CUPE $47,576 - 52,307 (2001)
