Program Facilitator--Regional Self-Management Program, Langs

Langs is the host organization for the Regional Self-Management Program.  This program is located in Cambridge, Ontario but services the region of Waterloo Wellington.

Program Facilitator- Regional Self-Management Program (20 hours/week, permanent)

Qualifications:

  • A regulated health care professional currently licensed, registered, or certified according to the requirements of the profession in the Province of Ontario;
  • Minimum of 3-5 years chronic disease management experience
  • Demonstrated understanding of principles of self-management and self-management support
  • Demonstrated leadership skills in supporting clinical practice change, education and professional development
  • Knowledge of adult learning principles;
  • Ability to work well with others, build teams, foster an environment of innovative problem-solving, conflict resolution and collaborative relationships with all stakeholders;
  • Excellent interpersonal, verbal and written communication skills;
  • Experience in presenting, promoting programs, and public speaking;
  • Strong knowledge of computer systems (MS Office) and website content management systems;
  • Proven ability to work independently, multi-task and meet deadlines;
  • Previous training in Self-Management Support Programs preferred
  • High level of initiative, judgment and discretion;
  • Attention to detail and efficient time management skills

Position Duties

  • Develop and support quality leaders through professional development activities such as workshops, refresher days, leader evaluation
  • Participate in recruitment and selection of leaders
  • Complete fidelity checks with the leaders
  • Collaborate with health care organizations to offer and support self-management strategies to support individuals and health care providers
  • Offer various presentations to health care professionals, and stakeholders to promote the program
  • Participate at regional and provincial networks or meetings
  • Lead workshops as available
  • Communicate and provide support to trainers, volunteer workshop leaders
  • Support communities of practice
  • Support mentoring and follow-up for health care professionals attending workshops
