The Approaches to Community Wellbeing is a regional resource to 31 First Nations Communities. The Approaches to Community Wellbeing provides expertise, support, services, and programming in public health to the region. The program also supports communities in the development, implementation, and monitoring of their own Approaches to Community Wellbeing. The Raising our Children section of the model includes Family Health, Youth Development, and Building Healthy Relationships.
The Community Wellbeing Nurse Specialist will work with the Raising our Children team to provide expertise around youth health and school year. The role of the nurse will be to support school staff, community-based workers, and to enhance community health services.
- Current Registration with the College of Nurses of Ontario, in good standing
- Baccalaureate Degree in Nursing BScN from an accredited University with identifiable community health nursing content
- Current CPR certification
- Experience in public/community health nursing, program planning, implementation and evaluation
- Knowledge of and/or experience with Youth Health, School Health and Mental Health promotion programs
- Experience with youth, children, and/or families and experience/interest working with at-risk families
- Knowledge and sensitivity to First Nations culture as well as the government health policies and regulations as they apply to First Nations
- Experience and training in First Nations cultural competency
- Ability to work collaboratively as part of a multidisciplinary team and foster relationships with stakeholders within SLFNHA and the communities
- Excellent interpersonal, critical thinking, and problem solving skills
- Demonstrated organizational and time management skills
- Excellent written and verbal communication skills with individuals and groups, including demonstrated ability to facilitate and conduct presentations, and write reports
- Ability to speak fluently in one of the Sioux Lookout Zone First Nations dialects, and or write in syllabics is an asset.
- Proficiency in areas of documentation and Microsoft Office suite skills including Word, Power Point and Excel database applications
- Must be willing to travel
- Valid driver’s license and access to a vehicle is required
Roles and Responsibilities
- Identify health issues and needs relevant to communities and participate in population level health planning
- Assess, plan, implement and evaluate community-based youth, school health, and nursing programs initiatives that address the identified needs of the community (e.g. healthy youth development, parenting, bullying prevention, increased physical activity, healthy relationships, healthy sexuality, building healthy school communities, substance use prevention, injury and violence prevention) using the current knowledge of practices and trends related to the field of public health
- Participate in the development and evaluation of education materials and provide support to health promotion activities for Raising our Children
- Support and build capacity of community-based health and youth workers through:
- Developing and providing resources
- Facilitating access to professional development opportunities
- Conducting training and workshops
- Support community-based and school-based nurses to provide health services for youth
- Support Youth Facilitators to provide Youth Development and Building Healthy Relationship programming and services at community and regional schools
- Conduct community visits and school visits to provide health education and clinical services for youth
- Provide clinical expertise to the Raising our Children team and to community health care providers
- Establish and maintain close working relationships with each First Nation community’s leadership, health staff and school staff in order to build capacity and promote awareness and self-determination within Youth Development and Building Healthy Relationships programming and services
- Facilitate community development and community mobilization
- Facilitate relationship building between youth, Youth Facilitators, schools, parents, Elders, and the wider community
- Provide orientation of SLFNHA’s Approaches to Community Wellbeing and Raising our Children Team to primary health care providers, and facilitate continued learning opportunities regarding youth health.
- Advocate for clients and communities to enhance health determinants
- Liaise and work in partnership with internal and external committees, working groups, task forces, organizations and coalitions as a representative for SLFNHA, to accomplish the roles and responsibilities noted above
- Conduct quality control surveillance on data management and systems operations in conjunction with the Raising our Children Manager, data clerks and the Epidemiologist
- Maintain required professional memberships and professional competence through participation in professional development programs and by self-directed study
- Perform other duties for the SLFNHA as directed
