I’m writing to inform you of a population health panel we’re putting together for CAHSPR2018 and to invite those of you who work in this area to express interest and potentially join the panel.
We’re looking for individuals and groups who work/research in the area of population health to participate in a conference panel tentatively titled "Increasing the scale and adoption of population health approach: examples from Canada and beyond.”
We are searching for a variety of examples from conceptual explorations and measurement to collaborations and practical application of population health approach or its elements (e.g., equity, social determinants of health, health and wellness). We want to present a mix of case studies, best practices, and examples and share more broadly ways in which interested individuals from across the continuum of care, government and academic/research institutions can reach out to foster partnerships, create and share tools, and start up working collaborations.
