Brief Action Planning (BAP) is a highly structured, client-centred self-management support technique. Composed of a series of 3 questions and 5 skills, BAP can be used to facilitate goal setting and action planning within the limited time available for appointments/interventions.
Objectives, at the end of this workshop, participants will be able to:
- Identify clinician’s key role as partners and facilitators in self-management of chronic conditions.
- Implement specific and efficient communication strategies that can be utilized to support client-centered care.
- Demonstrate how BAP can help patients experiencing ambivalence and/or low confidence to set achievable action plans and problem solve barriers to change.
This free workshop is provided by the Central West Self-Management Program in collaboration with the Center for Collaboration, Motivation and Innovation.
