Title: Registered Nurse, Certified Diabetes Educator
Reports to: Manager, Napanee Area Community Health Centre
Position Type: Permanent, Full Time (35 hours per week)
Expected Start Date: March 2017
Salary: $54,710 - $65,916 per year
The Registered Nurse, Certified Diabetes Educator, (RN CDE) provides community-based nursing care functions in the prevention and management of diabetes. They participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community that are affected by diabetes. The RN CDE works closely with the Registered Dietitian CDE and others to achieve a successful regional Diabetes Education Program. The RN CDE acts as an ambassador for the Napanee Area Community Health Centre in the community and promotes positive partnerships with other providers to maximize the capacity for diabetes interventions in South East Ontario.
This specialized Registered Nurse requires a Certified Diabetes Educator certification, or previous experience with Diabetes Education and willingness to complete certification.

Key Responsibilities / Detailed Responsibilities
- Delivers, or assists with the delivery of, prediabetes and diabetes education programs throughout Napanee and Area, in various community setting where the demand exists;
- Collaborates with partner agencies/hospitals/any community setting where demands exist in delivery of prediabetes and diabetes education groups;
- Educates, counsels and provides information to clients and/or programs within scope of practice, e.g. self-management strategies such as blood glucose monitoring;
- Involved in the delivery of insulin initiation and management;
- Aware and open to the implementation of alternate forms of treatment available to assess and treat clients;
- Assists in reviewing resource packages available for clients and adapts as necessary;
- Identifies the need for translation of materials to meet client needs;
- Acts as a health advocate for clients
- Provides home visiting services to clients as needed
- Facilitates access to diabetes information, education & support;
- Participates in diabetes screening events through collaboration with CHC/CRC’s, CDA, Public Health and other agencies;
- Participates in media events where required;
- Organizes events and meetings with community-based health providers to keep them informed of program services and updates;
- Provide primary care to clients, which takes into account the social determinants of health, in a manner helpful and acceptable to clients;
- Work as a part of an interdisciplinary team, including close collaboration (e.g., shared appointments) with the diabetes dietitian to offer optimal care for clients to ensure clients receive comprehensive, competent and appropriate health care;
- Screen, assess and provide appropriate referrals for clients, including those without appointments, prescribing routine treatment as appropriate (per Standards of Nursing Practice for Registered Nurses of Ontario);
- Provide client screening, assessments, and health education within scope of practice and in keeping with the Canadian Diabetes Association guidelines;
- Initiate, plan and facilitate groups to support clients achieve optimal health;
- Assist reception staff and other practitioners in screening client phone calls as needed;
- Provide telephone triage and client follow-up as needed;
- Provide on-going follow-up treatment services including communication through various means with clients, other staff, and external providers as appropriate
- Perform basic laboratory procedures, e.g. urine dipping, pregnancy tests, glucometer reading;
- Inform patients, either in person, or by telephone, of lab results and other information relating to their health;
- Administer first aid as appropriate for clients and staff as needed.
- Complete appropriate and up to date chart documentation in the Electronic Health Records for both individual and group encounters and support Quality Improvement initiatives by documenting symptoms and conditions, nursing interventions and referrals and recommendations for ongoing treatment;
- Receive and track referrals from staff and self-referrals from clients; maintain schedule and wait list;
- Monitor and adjust workflow to ensure program targets are met;
- Participate in chart reviews and case conferences;
- Identify areas where development of protocols and procedures are needed to improve client services or to promote more effective staff functioning;
- Ensure laboratory procedural forms are filled out correctly and that specimens are procured and handled appropriately;
- Maintain laboratory and procedures room, including developing and implementing procedures for sterilization, medical equipment organization, and medical supplies and drug ordering and stocking;
- Monitor controlled substances according to the policies and procedures of the NACHC and Kingston Community Health Centres
- Complies with all relevant legislation and KCHC policies, including privacy laws
- Supports consistent application and development of KCHC policies and procedures
- Supports KCHC’s student and volunteer placement programs
- Promotes awareness of and participation in KCHC activities
- Demonstrated commitment to continuous learning and quality improvement
Basic Education and Experience Requirements
- Undergraduate degree in nursing from a recognized university or combination of nursing diploma and additional relevant courses/training;
- Current registration as a Registered Nurse with the College of Nurses of Ontario;
- Certified Diabetes Educators (CDE) certification or commitment to achieve certification in a timely manner;
- Demonstrated experience working with people who have diabetes, preferably for minimum 3 years;
- A minimum of 3 years of relevant community nursing experience, preferably working collaboratively with other health care professionals to serve clients with complex needs.
- Experience in program development, implementation, monitoring and evaluation;
- Experience working in rural areas, with Indigenous health issues, and seniors are assets.
Knowledge, Skills and Abilities
- Proficiency in the use of computers and various software applications;
- Awareness of, and ability to support, provincial and federal privacy regulations.

- Valid Driver’s License, valid insurance coverage, and access to vehicle is required as this position has weekly travel to rural areas.
- This position requires regular evening work once a week and occasional work on other evenings or weekends.
