HC Link (http://hclinkontario.ca/index.php) supports community groups, organizations, partnerships and networks across Ontario that are working to create healthy, vibrant communities. We provide consulting services, learning activities and educational resources, and assist with community engagement and partnership development.
In order to provide groups with the best possible services, we need to hear from you about how satisfied you are with our services, and how else we could support your work. Your feedback will help shape our services over the next couple of years.
As someone who has been in touch with HC Link over the past couple of years, we would appreciate it if you would take about 20 minutes to complete the HC Link 2015 survey. We would like to hear from anyone who is involved in creating healthier communities, from diverse areas and sectors, even if you have never received a service from HC Link.
The link to the survey form is below. We would appreciate it if you would complete the survey by October 30, 2015. If you are not the best person to respond to this survey, please forward it to the appropriate person. You can also circulate this message to your members and networks.
You may leave the survey and re-enter it later to finish it. Your answers will be kept confidential and will only be reported in summary form. Some quotes from your responses may be used anonymously in HC Link reports and promotional materials, but any identifying information will be removed.
The survey may be completed in either French or English.
