Quest Community Health Centre is a not-for-profit, interdisciplinary, primary health care organization. We are one of 73 Ministry of Health and Long Term Care funded Community Health Centres. Our holistic interdisciplinary model of care focuses on primary health care, illness prevention, health promotion, community capacity building, and services integration, while taking into account the underlying social determinants of health such as poverty, housing, and unemployment.
With your skills, positive outlook and expertise, you are a catalyst who will help support individuals who do not have a primary care provider and who commonly experience barriers to accessing healthcare.
An experienced team builder and manager, you work within the context of the Centre’s mission and strategic directions to provide leadership in the delivery of integrated and responsive primary health care programs. These are delivered by an interdisciplinary team in a collaborative manner, with an emphasis on health promotion, education and community capacity building. You provide day-to-day support, coordination and guidance.
Your qualifications include experience in the development and management of programs preferably in a primary health care environment, working with inter-disciplinary teams. A Masters' Degree in Social Work, Health Promotion or Nursing is an asset. Knowledge of local community resources and referral processes, and the ability to work with diverse individuals and groups on complex community issues is required. A valid Ontario driver’s license, access to reliable transportation and a Police Reference check are also required.
