The Greater St. Catharines Community Health Centre (GSCCHC), located in the heart of the Niagara Region, is dedicated to serving the homeless and/or at risk of homelessness, individuals with mental health and addictions, isolated seniors, sexual and gender diverse populations, at-risk children and youth. A not-for-profit, interdisciplinary, health care organization, we are one of 21 new government funded community health centres. Our holistic model focuses on primary care, illness prevention, health promotion, community capacity building, and services integration.
With your skills, positive outlook and clinical expertise, you are a catalyst who will help support a new environment for residents who do not have a primary care provider and who commonly experience barriers to accessing healthcare.
You will work in collaboration with Physicians, Nurse Practitioners, Health Promoters and other allied healthcare professionals. And, you can make a special contribution while enjoying the opportunity for continuous learning and career growth.
Your qualifications include 3-5 years experience in a community setting working with inter-disciplinary teams. Proficiency in current psychosocial theory and Brief, Solution Focused counseling approaches. Experience working with both individuals and groups, possessing relevant support group management. Membership in good standing with Ontario Association of Social Workers or OCSWSSW is an asset; Masters degree in Social Work is preferred. A valid Ontario driver’s license, access to reliable transportation and a Police Reference check
